At Connect Transitional Care Management, we are only as strong as our staff and are proud to employ a variety of qualified individuals who bring their own diverse knowledge to the practice in order to ensure we provide the most comprehensive treatment possible.

The Health Care industry often talks of “Continuity of care”.  We all say, “The patient comes first”; Who can disagree?  All too often processes, laws, procedures, etc. stand in the way of providing the type of patient care we all want to provide.  CONNECT has unlocked the path that makes continuity possible as the Clinic, Home Health and Hospice Agencies, Care Centers and Hospitals join in an atmosphere of  “Co-Opetition”.  Each providing their own unique care, working seamlessly with one another, staying connected with the One (singular) patient.
