CONNECT Transitional Care Management, LLC is full service Clinical Case Management. CONNECT enables the PCP to establish and maintain contact with the patient from admission to discharge in the following facilities: Hospitals, Home Care Services, Skilled Nursing Facilities, Rehab Units, and Assisted Living Facilities. CONNECT’s slogan is “Specialized in Service, ONE in Purpose”. The ultimate goal is to connect the Patient to their PCP and Community Resources. We are confident this process assists in ensuring continuity of care.

CONNECT has shown a high rate of return by increasing revenue for Participating Physicians and Clinics by capturing CPO and TCM data, complying with and adhering to CPO and TCM Medicare Guidelines, and properly submitting this data to billing. These actions help decrease the cost of Medicare funds by reducing re-hospitalizations.

CONNECT is able to give and take Verbal Orders, set up Home Service Evaluations, and upload Post Visit Notifications for CPO’s by utilizing the scope

of practice afforded to Registered Nurses. RN's also oversee TCM within the required guidelines, insuring all billing codes are properly submitted. All of this is done electronically, drastically reducing paper flow. 

CONNECT ensures that the PCP’s and Home Service Providers are within compliance of Anti-Kickback and Stark Laws. Kelly Criddle, License and Compliance Manager of the Utah State Department of Health has reviewed all contracts and found this concept to be “genius” and "in the best interest of the patient”.
